Amlodipine is a widely prescribed calcium channel blocker that is generally well tolerated, with peripheral edema being its most recognized adverse drug reaction (ADR). Musculoskeletal complaints such as arthralgia and peri-articular swelling have been reported but remain exceedingly rare and poorly characterized. We report a case of recurrent joint pain and peri-articular swelling in a 64-year-old woman receiving long-term amlodipine 10 mg daily. Symptoms appeared several hours after each dose and resolved within 48-72 hours of drug discontinuation. No alternative causes were identified, and clinical evaluation excluded gout and inflammatory arthritis. Cessation of amlodipine resulted in marked symptomatic improvement, and antihypertensive therapy was switched to candesartan. The Naranjo Algorithm yielded a score of 7, indicating a probable ADR. This case highlights a rare musculoskeletal reaction, potentially linked to amlodipine-induced periaortic edema or tissue susceptibility, in older patients with multimorbidity. Early recognition of such atypical ADRs is essential to prevent unnecessary morbidity and to strengthen pharmacovigilance data.
